NHS Health Checks Programme Survey
Overview
Welcome to the NHS Health Check programme survey.
Everyone is at some risk of developing heart disease, stroke, diabetes, kidney disease and some forms of dementia. An NHS Health Check aims to help you lower your risk of developing these common but often preventable diseases. From April 2013, responsibility for the NHS Health Check programme moved to your local authority, which will invite anyone who is eligible to have a check over a five-year rolling cycle. That means everyone aged between 40 and 74 who hasn’t already been diagnosed with any of the conditions mentioned above will be invited for an NHS Health Check.
After the check individuals are signposted to various lifestyle services such as weight management, exercise and one to one support to manage their risk. The programme is free for all eligible participants. In Richmond, it is being delivered by participating general practices and pharmacies and at community outreach events held in various parts of the borough since 2009.
This survey is being conducted to understand people's views and opinions on the programme to ensure that the best service possible is provided.
